Best surgery type: Liposuction + gland excision + minor skin tightening. Advanced gynecomastia cases like Grade III require skin tightening and longer surgical time. Some surgeons use Vaser or Renuvion for skin contraction to avoid larger scars. 

Scar: Peri-areolar + lipo ports. May need small vertical scar if skin is borderline.

Grade 4 Gynecomastia: Severe Case & Skin Excision Surgery

What it looks like: Large, feminized breasts with significant skin excess and nipple drooping below the breast fold. Common after massive weight loss.

Best surgery type: Excision + liposuction + skin removal. This is “severe gynecomastia surgery” which involves complex glandular excision and skin tightening. May need free nipple grafts or anchor-scar reduction. 

Scar: Peri-areolar, plus anchor/inverted-T scars similar to female breast reduction. More scarring, but necessary for flat contour.

How Doctors Determine Your Gynecomastia Grade

1. Physical exam: Surgeon checks tissue type — fat vs gland — and measures breast projection.
2. Pinch test: Gland feels firm/rubbery; fat feels soft.

4. Imaging: Ultrasound or mammogram if cause is unclear, to rule out tumors.

Note: Pseudogynecomastia = fat only, no gland. True gynecomastia = gland present. Only true gynecomastia needs excision.


Can Gynecomastia Grade Change Without Surgery

Yes, but limited:

  • Grade 1 in teens: Pubertal gynecomastia often resolves in 2 years without treatment.
  • Weight loss: Can reduce Grade 2-3 if it’s mostly fat. Glandular tissue won’t shrink with diet.
  • Stopping triggers: Anabolic steroids, marijuana, certain meds can cause Grade 1-2. Stopping may help.
  • Grade 3-4: Skin excess never tightens fully on its own. Surgery is usually needed.

If it’s been >2 years and still present, it’s unlikely to go away naturally.
