The three keys to a healthy weight:

  1. Eat well

  • Don’t skip meals – this may lead to overeating later. Weight loss efforts are most successful when meals and snacks are small and spread out during the day.
  • Eat balanced meals. Choosing a wide variety of foods is important for good health, but control portion sizes and avoid second servings. Try serving the foods on your plate with these tips in mind:
    • Fill one quarter of your plate with lean protein – this portion size fits within the palm of your hand. Lean protein sources include fish (especially salmon), poultry, egg whites and tofu.
    • Then fill at least half of your plate with non-starchy vegetables like tomatoes, salads and other greens, broccoli, carrots, mushrooms and green beans.
    • If desired, add a small portion of fruit or a cup of low-fat or non-fat milk or yogurt. Or, enjoy these as a between-meals snack instead.
  • Limit foods that are high in saturated fat, since fat is a concentrated source of calories.
    • Choose lean meats. Trim visible fat from meats and skin from poultry before eating.
    • Use lower-fat cooking methods, such as baking, broiling, roasting, stewing and steaming. Avoid frying foods.
    • Watch out for added fats from condiments, such as cheese, butter, margarine, mayonnaise and salad dressing.
    • Eat less added sugars in the form of table sugar, honey, sweets, pastries and desserts.
    • Avoid high-calories beverages like sodas, fruit juices and alcohol.

 

  1. Be active – exercise

Try making exercise a daily habit. Aim for at least 30 minutes of moderate activity, such as brisk walking, on most days of the week. If your goal is to lose weight, however, you’ll benefit from doing more, as much as 60 to 90 minutes of activity per day.

Don’t set the bar too high when you’re just getting started. Start slowly, and gradually increase your daily activity level. Some tips for increasing activity levels:

  • Limit TV, computer and video game use.
  • Schedule family exercise time.
  • Get an exercise video or equipment to use at home.
  • Walk the dog.
  • Do errands by foot or bike.
  • Take the stairs instead of the elevator.
  • Park farther away or get off the bus a few stops early and walk the rest of the way.
  • Join a walking group, aerobics class, sports team, local pool, gym or community centre.

 

To stay motivated

  • Choose a variety of activities to keep your workouts interesting.
  • Set realistic goals, at a reasonable pace.
  • Plan exercise into your weekly schedule.
  • Find an exercise buddy – you can rely on each other for support.
  • Keep an exercise log. Writing down what we do helps keep us focused.
  • Set a non-food reward for progress in meeting your goal.

 

  1. Be aware and plan

  • At home, eat only while sitting down at the table and stay out of the kitchen unless you are preparing food.
  • Keep tempting foods out of sight and make healthy snacks available.
  • Use smaller plates, bowls and glasses.
  • Make it inconvenient to eat more by chewing gum or brushing your teeth after meals.

Eat mindfully

  • Take small bites.
  • Chew your food well.
  • Put your utensil down between bites and savour the taste of the food.

Monitor your progress:

  • Check your body weight at least once a week – daily is okay – and keep a logbook.
  • Keep a daily food journal. It will make you think more carefully about what you’re eating and allow you to monitor calories. There are many ways to track calories, including applications for your handheld device, free or paid online programs, or the good old fashioned paper and pencil method.

What is Alopecia Areata?

Alopecia areata is an autoimmune condition where the body’s immune system is attacking the hair follicles, causing the hair to fall out.  The natural history of the condition is usually unpredictable with patches of hair loss occurring, followed by regrowth and then potentially further patches of hair loss occurring.

It is often referred to in the community as just ‘alopecia’ however this term should always be qualified with the word areata as there are other forms of alopecia such as scarring alopecia, androgenetic alopecia etc. The terms totalis and universalis are descriptive terms referring to loss of the entire scalp hair and loss of the hair on the entire body respectively.

Therefore, one would refer to these conditions as alopecia areata totalis or alopecia areata universalis to signify that these are particular presentations of the one condition, rather than a distinct entity.

Fortunately, the autoimmune-induced inflammatory response does not cause destruction of the hair follicle and so as the follicles are still present regrowth is always possible.  This differs from some of the other hair loss conditions.

 

HOW COMMON IS ALOPECIA AREATA?

Alopecia areata affects approximately 4% of the population (90,000 in the UK alone), and around 98% of those diagnosed recover. Alopecia areata can affect all ethnic groups and ages, but is most likely to occur between the ages of 20 and 50.

It also occurs equally in both men and women.

 

WHAT DOES ALOPECIA AREATA LOOK LIKE?

The main diagnostic feature of alopecia areata is the presence of bald patches surrounded by little ‘exclamation mark hairs’.

They are thicker at the top and taper to a thinner end, with the root a blob at the bottom.The number of exclamation mark hairs you have can indicate how active the alopecia areata is. In general, the more there are, the more rapid its progress. However, sometimes there are no visible exclamation mark hairs at all. In these cases, alopecia areata may be diffuse (spread over a large area), but this is rare.

In alopecia there is also inflammation present at the lower end of the hair follicle, but you can’t see it. Rarely, small patches of alopecia areata can grow, overlap and progress to total baldness, or alopecia totatlis.

 

CAUSES OF ALOPECIA AREATA

Alopecia areata is widely accepted to be an autoimmune disorder, where your body sees certain hair cells as foreign enemies and attacks them. What triggers this response isn’t entirely understood, but 90% of cases are associated with stress, shock, bereavement or an accident and/or illness.

There is also often a genetic predisposition and there are in fact several reported cases of alopecia areata’s occurrence in twins.  Alopecia areata is also more common in those that suffer from hayfever, eczema, Addison’s disease, pernicious anaemia, rheumatoid arthritis, ulcerative colitis, lichen planus, diabetes mellitus, vitiligo, lupus and thyroid disease.

 

TREATMENT

In many cases, alopecia areata spontaneously resolves itself within a year, but there are also alopecia areata treatments available to help bring about remission.

 

CORTICOSTEROID INJECTIONS

Corticosteroid injections are thought to be the most effective of treatments for alopecia areata, especially when used on small patches. They are given every few weeks, and can be used to treat bald areas on your scalp, eyebrows and body.

They suppress your body’s immune system, so that it no longer attacks your hair follicles.

 

SIDE EFFECTS

TOPICAL CORTICOSTEROIDS

FUT stands for follicular unit transplantation and is otherwise known as “strip surgery”. This involves removing a piece of skin, dividing it under a microscope into individual groupings of follicular units and then those are transplanted into the recipient area.

FUE stands for follicular unit excision. It used to stand for extraction, except excision sounds more like the sort of thing only a doctor should do and so it was changed. Rather than a strip, FUE involves “individual follicular extraction in which follicles are removed within a small circular punch that is less than a millimetre in diameter,” both types have merit and both should be considered as both options have long been tested and proven.

There is, realistically, only a handful of surgeons who perform very good, state of the art transplantation surgery and who utilise the right hairs in the right place. A lot of clinics are trying to open up, cutting corners and quality in an attempt to profit off the increased interest in surgery thanks to celebrity examples.

The cost for a Hair Transplantation by doctors of proper credentials and experience is anything between RM10,000 and RM25,000 (USD3,000 to USD6,000) depending on the severity of the hair loss issue, the donor area and the option chosen for transplantation. Anything cheaper is a warning sign. Many deaths and permanent damage have occurred in cheap transplantations, they are cheap for many unscrupulous reasons.

Always be reminded that the doctor’s credentials and experience should be of top importance as you place your life and safety in their hands. You need to be prepared to pay for quality and assurance to life, safety and hygiene.

 

Follicular Hair Transplant

Considered as gold standard, Follicular Hair Transplant can be considered as one of well-known hair restoration surgery. Hair grows from the scalp in groupings called follicular units which contain sebaceous unit, supporting structures while containing 1 to 4 terminal hairs. The hair transplant surgeon utilizes the follicular unit for faster healing, for greater density and fewer traumas to existing hairs in the recipient area. The redistribution gives the appearance of fuller look while the number of hair may not change.

 

Follicular Unit Extraction

With small punch excisions ranging from 0.8 to 1.5mm, in FUE method follicular units are harvested separately and then manually removed with forceps. Due to the difficulty in determining the angle and depth of hair follicles, the hair transplant surgeon’s initial experience yielded approximately 30% transaction rate.

The transaction rate at this stage is as low as 6% with improved instrumentation and better understanding of follicular unit. Where grafts are removed and thinning of donor area can be seen, if too many grafts are detached, hyper pigmentation can occur.

Patients who are prone to hypertrophic scarring or those who have undergone strip method, FUE method can be perfect for patients who have little scalp laxity. For a more natural appearance the hair can be placed in the scalp after extracting from the donor site. While the strip method is generally reserved for the posterior part of the scalp, FUE can also be used to harvest hair follicles from other parts of the body.

According to the number of terminal hairs, the assistant then takes grafts and separates them before placing them in the bald area of the scalp, while trimming is rarely required.

 

Platelet Rich Plasma

In order to increase hair yield rate, diameter and hair density, PRP (Platelet Rich Plasma) can be described as an emerging trend in restoration of hair.

This therapy takes the blood from the arm of the patient while putting the blood into the centrifuge and while spinning them to separate out red blood cells from the plasma. Then this plasma is injected directly in the scalp at the level of hair follicles.

You have to take people back a few steps, because people need to know what’s going to happen to the rest of their hair. What to look for in a doctor

  1. Who is doing the surgery?

If the person you’re talking to can’t tell you who the doctor is going to be that’s an alarm bell, as they might be flying in an unlicensed, unaccountable doctor or in most cases, a doctor with no proper Hair Transplant credentials.

 

  1. Look for quality, rather than quantity, from a doctor.

A doctor with credentials who personally performs the surgery which can take a full day.

 

  1. Is the procedure done by the doctor?

Cheaper clinics may aim to have nurses or technicians perform the entire procedure. The cutting of skin and the injecting of the anaesthetic, the planning of the hairline, seem like simple things, but in reality you’ve got to be able to plan something that works for the rest of the patient’s life.

The surgery

The donor area is checked for strength and density, doctor will advise the transplant of grafts over a one-day procedure, lasting eight hours. The big day will start off with having the head shaved. After the area is cleaned and given local anaesthetic, the doctor will begin with the hair transplant procedure.
