What is Fat Transfer?

Fat transfer is a surgical procedure that transfers fats taken from the patient’s body to areas of the face and body that require fats in order to restore the volume lost because of the process of aging. The procedure is also called fat grafting or fat injection. Typically fat is transferred to the breasts, the buttocks, hands and facial region including the lips, chin and cheeks.

 

How is Fat Transfer done?

Fats are removed from one area of the body by the surgeon using liposuction. The fat is purified and turned into liquid form. It is then injected into another part of the body to give more volume. Water assisted liposuction is the latest and safest method of liposuction being used by plastic surgeons today as it holds no risk as traditional liposuction. As in all types of liposuction, fatty deposits are removed from beneath the skin for a more contoured look. Liposuction is optimal for people who are in good physical shape.

Instead of injecting the body with tumescent fluid and removing the fat cells manually with a cannula, or using laser technology to melt and dislodge the fat, the water assisted liposuction actually uses a slightly pressurized stream of saline to dislodge fat and simultaneously remove the cells. Instead of destroying fat cells, this type of liposuction loosens the fat cells to facilitate more gentle removal. Water Assisted liposuction injects tumescent fluid (saline and medicines including a local anaesthetic) into the body part being treated. The Procedure

How Liposuction Works

Liposuction is an outpatient procedure and can be safely performed using local anaesthesia, intravenous sedation, or local anaesthesia, depending on the nature of the procedure. Your cosmetic surgeon will recommend the anaesthesia method that is most appropriate for your needs.
Your cosmetic surgeon will make one or more small incisions near the treatment area. A cannula, or thin tube, is inserted through these incisions to loosen excess fat beneath the skin. Then, the fat is suctioned out. The entire procedure can take under an hour or up to 3 hours or longer, depending on the number of areas treated.

 

Liposuction Recovery

Today’s advanced liposuction techniques are designed to minimize swelling, trauma and discomfort, and when the procedure is performed by a qualified cosmetic surgeon, recovery from liposuction can be remarkably quick. Many patients return to work just a few days after their procedures, depending on the physical requirements of their jobs and the extent of their procedures. While your cosmetic surgeon can prescribe pain medication, most patients find over the counter options more than enough to manage any post-operative soreness.

Your cosmetic surgeon may give you a compression garment to wear for a few weeks following surgery. This helps to minimize swelling and encourage optimal skin contraction. Full results will take at least four months to reflect.

The Compression garment is not an option but compulsory for 12 hours in a day for the next one month. Wearing your compression garment will help reduce the swelling and hasten the healing process. No exercises are allowed for one month after the procedure. Please keep in mind that final results may take six to twelve months and one is expected to live a very healthy lifestyle and maintain a strict diet after the liposuction procedure. Please take note that for those who experience loose skin after liposuction. Patients will be advised to purchase a course of Laser Skin Tightening sessions.

As with Fat Transfer procedures, this can be done immediately during the liposuction and yes, the full results will take four months to reflect and patients are advised to consume healthy fats such Avocados, Cheese and Full Cream Milk.

As for a Tummy Tuck procedure, it can only be done four months after the liposuction procedure. Patients must be aware that in most cases, a Tummy Tuck procedure would require a liposuction of the abdomen and flanks and then followed by a Tummy Tuck procedure four months later. Buccal fat extraction, buccal fat excision, buccal fat removal involves carefully removing defined amounts of fat tissue from the buccal fat pads in the cheeks. Buccal fat removal is a concept of modifying the deep fat of the face from inside the mouth, is a gentle, precise and tissue-sparing procedure for facial contouring and rejuvenation. The surgery is effective for improving facial proportions, soft-tissue asymmetries and age-related lower facial heaviness.

Buccal fat reduction is the process of surgically removing the fat pads within the cheek. This fat sits on either side of the cheek and under the cheekbones. This fat can remain even after a person loses weight, and is often perceived as giving someone a child-like appearance. As such, a buccal fat reduction is often an appealing choice.

Facial fat is composed of multiple fat compartments, arranged in deep and superficial layers. They are separated by connective tissue, forming well-defined anatomical units – known as ‘fat pads’. Their function is to provide insulation and structural support throughout the facial soft-tissues.

The adipose compartments change throughout life, some are prone to deflation (fat atrophy), whilst others may gain in volume (fat hypertrophy). These volumetric changes are reflected in the contours of the face and will determine how individual regions develop with ageing.

The buccal fat pad is a deep fat compartment that extends from the upper mid-face to the lower face (next to the mouth). Due to the elongated shape, it is prone to a downward volume-shift within the compartment and can in some instances form a small hernia. This may result in fat accumulation and potentially a bulge in the lower cheek, next to the mouth.

 

Slimming and contouring the face

A reduction of the buccal fat pad will result in slimmer contours of the front part of the lower face. The treatment highlights the natural definition of the underlying craniofacial architecture, providing a more defined jawline and cheekbones.

Buccal Fat Removal may be indicated as an ancillary procedure to a SMAS Facelift – reducing age related lower facial heaviness.

 

Facial Proportions

Modification of the Buccal Fat Pad can be performed as a stand-alone treatnment to remove excess fat from the lower cheeks. The procedure is also used for balancing congenital or acquired soft-tissue asymmetries (for example the hemifacial changes associated with facial palsy.

How long does the procedure take?

The procedure lasts one to two hours depending on the extent of the enhancements. You may experience swelling immediately after the procedure and for at least three days.

 

Local Anaesthesia

Local Anaesthesia will be administered. Your surgeon will make an incision about two to four centimetres long between the cheek and gums inside the mouth. The incision starts above the second upper molar towards the back of the mouth. The surgeon uses blunt dissection to expose the fascia and fibres of the buccinator muscle. Your surgeon will then use the pressure of his or thumb externally against your cheek right under your cheek bone causing the buckle fat to protrude through the incision. He or she will then “tease” the fat out with forceps a little bit at a time. Non-dissolvable sutures close the incisions. There may be slight differences in surgical technique depending upon the preference of your surgeon.

 

Recovery Period

Most patients recover fully within three weeks, however, recovery rates vary and are influenced by your general health, nutrition and the extent of the procedure.

Water Assisted Liposuction

Water assisted liposuction is the latest and safest method of liposuction being used by plastic surgeons today as it holds no risk as traditional liposuction. As in all types of liposuction, fatty deposits are removed from beneath the skin for a more contoured look. Liposuction is optimal for people who are in good physical shape.

Instead of injecting the body with tumescent fluid and removing the fat cells manually with a cannula, or using laser technology to melt and dislodge the fat, the water assisted liposuction actually uses a slightly pressurized stream of saline to dislodge fat and simultaneously remove the cells. Instead of destroying fat cells, this type of liposuction loosens the fat cells to facilitate more gentle removal. Water Assisted liposuction injects tumescent fluid (saline and medicines including a local anaesthetic) into the body part being treated. Tumescent fluid is proven effective and several alternate liposuction technologies are now using the tumescent technique for its local anaesthetic benefits. In the case of water assisted liposuction, it then uses different processes to actually remove the fat and manage fluid levels.

 

The Procedure

Once the local anaesthesia has taken hold, a cannula is then introduced into the tissue to irrigate and then simultaneously suction the loosened fat cell from the body. Finally, any remaining fluid is removed. The procedure is considered gentle and minimally invasive and the force seen in traditional liposuction methods is not needed when using jet-assisted liposuction. There is usually very little bruising or swelling and water assisted liposuction often has a very short recovery time, partially because the use of water means that there is less tissue trauma to the body.

 

No General Anaesthetic required

This is another liposuction technology that does not require general anaesthetic but only a Local Anaesthetic (LA), which drastically reduces risks to the patient and significantly reduces costs. Also, because of the fluid management used in this technique, the surgeon can do a more detailed anatomical assessment while conducting the procedure, so the results are excellent. Basically, the area of the body doesn’t swell up, so the plastic surgeon can actually see the results of the liposuction as he or she is doing it.
