UPPER BLEPHAROPOLASTY

UPPER BLEPHAROPOLASTY / EYELID SURGERY

What is Upper Blepharoplasty?

Also known as upper eyelid surgery. Is a procedure to remove extra skin and tissue to improve vision or cosmetic appearance. Eyelid surgery in Malaysia is very common. Here at Glojas, we are listed for the double eyelid surgery KL clinic and we emphasize patient’s safety.

Is it painful?

It is done under local anaesthesia, meaning you are fully awake but you will receive medicine to numb the area and block the pain.

How does it work?

The surgeon will make an incision around the area and remove the excess skin and fats. A thin stitch is then used to bring the skin together to allow for the creation of an eyelid crease.

How long is the recovery?

There will be minimal bruising and swelling for at least 7 to 14 days. Cold packs at home will help to make the swelling subside faster.

Will there be after care?

You will be given medication for swelling, pain, antibiotic and also additional antibiotic cream and eyedrops. We suggest you bring dark sunglasses for you to wear outside after the procedure and you will be back after a week to remove the stitches.

How much does it cost?

It starts from USD1,000 onwards and solely depends on the issue and desired results.

Upper Eyelid Surgery

Upper eyelid surgery can rejuvenate puffy, sagging or tired-looking eyes by removing or redistributing excess fat, skin and muscle from the upper eyelid. Also known as upper blepharoplasty, it may be performed for purely cosmetic reasons, or to improve sight by lifting droopy eyelids out of the field of vision. The surgery aims to recreate a fresh, open and youthful appearance.

There is a wide range of possible procedures performed under local anaesthesia. Incisions are placed either in the natural lines above the eye, providing a quickly fading scar, or inside the lid, resulting in instant invisibility.

The eyelids are among the most delicate structures in the face, composed of fine layers of soft tissue and the thinnest skin in the human body (eyelid skin being less than 1 mm thick, with sparse underlying subcutaneous fat). This relative absence of a padding fat layer predisposes to the characteristic skin laxity and fine wrinkles that appear with age.

Volume enhancing eyelid surgery (tissue-sparing blepharoplasty)

Volume-enhancing upper eyelid surgery or ‘tissue-sparing upper blepharoplasty.’ Unlike traditional techniques which alter volume proportions between fat and muscle (and may have negative effects on ageing in the long term), this method reconstructs and maintains a person’s previous youthful appearance by repositioning rather than removing tissue. It is our In-House Plastic Surgeon, Dr.Mohd Ali’s opinion that this approach best considers the aging process in this area and delivers results that are the most rejuvenating and long lasting.

Lower Eyelid Surgery

Lower eyelids may suffer from loss of skin/muscle tone and fat accumulation over time, which can cause bulging beneath the eyes and hence the formation of eye bags. Surgery (known as lower blepharoplasty) helps rejuvenate lower eyelids by relocating or removing excess fat, and where indicated, tightening and strengthening loose or sagging tissue. The results are softened wrinkles, tightened skin and improved eyelid function.

The lower eyelid with associated Canthal Ligaments and Cartilage (Tarsal Plate) is among the finest and most complex of facial structures. It protects the eye and helps to keep the eyeball moist and healthy.

Muscle-conserving lower eyelid surgery

Though there are several approaches to the lower eyelid, the optimal strategy is to spare or redistribute tissue rather than remove it, and to retain fullness of the lid and the periorbital area.

For most cosmetic lower eyelid surgery, Dr.Mohd Ali employs the Transconjunctival approach, a technique which involves cutting along the inside surface of the eyelid rather than through the outer skin. Fat is then conservatively removed or repositioned to restore eyelid fullness. This method of access conserves the structural integrity of the eye muscle and nerves, leaving motor function intact. Furthermore, the healing process is enhanced and it results in no visible scarring of the skin.

Several options for treating loose skin may be considered along with Transconjunctival eyelid surgery. These include microsurgical skin trimming, skin peel or laser skin resurfacing.

Canthopexy

Canthopexy can be one of the most important and rejuvenative components of lower eyelid surgery. It is a surgical tightening of the outer corner of the eye, which may become loose or rounded with age. The procedure can restore the shape of the eye and improve the tension of the lower eyelid. Canthopexy is a tissue-conserving procedure that reinforces the existing tendon without cutting or detaching it. The restoration of tension within the tendon and muscles reduces the amount of fat that requires removal, which helps prevent the future development of lower eyelid hollowness.

Canthopexy is frequently combined with a light surgical tightening of the Orbicularis Oculi muscle, as improved muscle tone will help raise sunken eyelid tissue and soften the border between the eyelid and cheek.

The Eyelid Corrective Surgery

The Eyelid Corrective Surgery at Glojas Plastic Surgery Centre is done by Dr.Mohd Ali who is our In-House Plastic Surgeon. Dr.Mohd Ali is one of Malaysia’s top plastic surgeons and Malaysia’s only fully qualified Craniofacial Surgeon. Dr.Mohd Ali holds more than 15 years of rich experience, knowledge and talent in Cosmetic surgery, Corrective surgery and Craniofacial surgery. Our Medical Centre is located in an affluent residential area based in the heart of Kuala Lumpur, Malaysia.

An eyelid surgery (Blepharoplasty) is an operation to remove excess skin and fat from your eyelids and to tighten your skin and soft tissues around your eyes. During surgery, incisions are made in the natural folds of the eyelid, in the crease of the upper eyelid and just beneath the lashes or behind the lower eyelid. This way, incisions are virtually unnoticeable after they have healed.

While you are lying down during blepharoplasty, your surgeon will make precise markings to designate where excess skin and fat pads need to be removed. Some underlying muscle also may be removed. These tissues are removed with surgical instruments such as scalpels, surgical scissors, radio-frequency cutting devices and sometimes cutting lasers. Our Plastic Surgeon will reconfigure areas around the eyebrows and eyelids.

During the procedure, your surgeon will make judgments about how much skin, muscle and/or fat to remove, based on a preoperative evaluation of factors such as your underlying facial muscle structure, bone structure and the symmetry of your eyebrows. Dry eye patients frequently require that less tissue be removed to avoid exposing more of the eye to the air, which can cause symptoms to worsen.

In cases where the eyebrows also are droopy, a procedure to elevate the eyebrows also may be appropriate. This procedure, called a brow lift.

WHAT ARE THE BENEFITS OF SURGERY?

Your face should look significantly younger and brighter. If an upper eyelid is interfering with your vision, your vision should improve.

WHAT DOES THE OPERATION INVOLVE?

An upper-eyelid blepharoplasty is usually performed under a local anaesthetic that is injected in your eyelids. A lower-eyelid blepharoplasty is performed under a general anaesthetic.

The operation usually takes an hour to 90 minutes.

  • Upper-eyelid blepharoplasty – Your surgeon will make a cut on the natural skin crease just above your eyelid. They will remove any excess skin and fat that is pushing through the muscle.
  • Lower-eyelid blepharoplasty – Your surgeon will make a cut along the rim of your eyelid below your eyelashes to just beyond the outside edge of your eye. They will remove any excess skin, and remove or redistribute any fat that is pushing through the muscle.

When the fat around the eyes is excessively accumulated or sunken, the eyes underneath appear dull and gives tired impression. In this case, we perform procedures such as Under Eye Fat Grafting to fill up more fat under the eyes, Under Eye Fat Removal to remove excess fat, and Under Eye Fat Repositioning procedure to even out the fat under the eye. To ensure excellent surgery result with high degree of completion, precise analysis must be made to consider the characteristics of each patient’s eyes and to carry out idea surgery method.

The eyes are the window to the soul. So when your eyes show signs of aging, excess skin, bags or wrinkles, the perception is that of being tired or old. Eyelid rejuvenation therefore is one of the most common plastic surgery procedures to improve the appearance of the face. Formally known as Blepharoplasty, eye bag removal is a plastic surgical procedure to refresh the eyes by removing excess skin in the upper and lower eyelids. Eye bags caused by drooping fat can also be corrected either by excision of excess fat or repositioning the fat. The aim of surgery is to achieve a refreshed, rejuvenated, natural appearance.

Blepharoplasty is a cosmetic surgical procedure to refresh the eyes by removing excess skin (hooding) and or fat (eye bags) to rejuvenate the eyes providing a youthful appearance. The eyes can give a tired appearance, hooded upper eyelids or puffy lower eyelids, eventhough you are well rested. Surgery aims to reverse or improve the appearance of the eyes to give a refreshed natural appearance. If the upper eyelids are very heavy, hooded, these can feel very tired and following surgery the eyelids feel more comfortable and refreshed.

Ptosis Corrective Surgery

Drooping eyelids (ptosis) can be a developing problem as people age or, indeed, a congenital one present from birth. Mild ptosis may not need treatment. However, if the ptosis is interfering with the vision, the consultant may recommend ptosis correction, through ptosis surgery. This surgery is performed by Malaysia’s ONLY Craniofacial Surgeon, Dr.Mohd Ali’s Medical Practice is based in an affluent residential area in the heart of Kuala Lumpur.

Ptosis Surgery for adults

Ptosis surgery typically involves the shortening of the muscles (tendons) that have the function of raising the eyelid. This muscle (tendon) is then reattached to the eyelid using sutures (row of stitches holding together the edge of a wound), which are hidden under the skin.

In some cases, the sutures (stitches) may remain slightly visible on the skin of the eyelids for one week after surgery, which is when they are typically removed.

This condition means that the border of either one or both of the upper eyelids droop to a lower position than is normal. This can therefore cause unwanted aesthetic problems, giving people the appearance of a tired look. It can also cause visual problems if the eyelid is unable to clear the visual axis and in some severe cases, the droopy eyelid covers the some or all of the eye, therefore affection vision.

Symptoms of Ptosis, Droopy Eyelid

The obvious symptom of ptosis is a drooping eyelid, which can affect one or both of the eyes. The severity of the condition varies from person to person, but typically patients will find it difficult to keep their eyelids open, have eye fatigue especially when reading, have strained eyes and suffer from eyebrow ache as a result of the great effort required to raise their eyelids. For some people, droopy eyelids may not cause medical issues but are simply a cosmetic issue.

Ptosis causes the narrowing of the eye opening, when means the eye appears smaller than it actually is. Moreover, if the condition is covering the pupils and therefore causing vision problems, the patient may find themselves subconsciously raising their eyebrows to help see better, which can lead to headaches and result in having a strange, surprised appearance.

It is important to seek immediate medical attention if the droopy eyelid:

  • Suddenly develops over a short period of a few hours or days
  • Droopy eyelid is accompanied with double vision, weakness in the facial muscles or arms and legs, difficulty speaking or swallowing, or severe headaches
  • There are also symptoms of eye infection, including pain and redness, fever, bulging eyes, or movement difficulty

Post-surgery

Patients will usually be able to go home after a few hours following the surgery, but children will need to stay in the hospital until the anaesthetic has worn off. Pain medication will be provided and an eye patch should be worn temporarily. It is not safe to drive home from the hospital, so be sure to arrange suitable transportation. Antibiotic eye drops will be prescribed for several weeks after the operation and should be used correctly to aid healing.
