What is eye bag removal?

Eye bag removal, known medically as the blepharoplasty procedure, is performed in order to produce a more youthful, fresh look to the eyes and overall facial appearance. The aim of the surgical procedure is to remove or reposition the fat and to smooth out the loose, wrinkled skin, which occurs as a result of ageing, sun damage, exhaustion and certain conditions, such as Thyroid Eye Disease. Eyebag Removal may be a lower or upper blepharoplasty and the procedure is considered minor.

Upper Eyelid Surgery 

Upper eyelid surgery can rejuvenate puffy, sagging or tired-looking eyes by removing or redistributing excess fat, skin and muscle from the upper eyelid. Also known as upper blepharoplasty, it may be performed for purely cosmetic reasons, or to improve sight by lifting droopy eyelids out of the field of vision. The surgery aims to recreate a fresh, open and youthful appearance.

There is a wide range of possible procedures performed under local anaesthesia. Incisions are placed either in the natural lines above the eye, providing a quickly fading scar, or inside the lid, resulting in instant invisibility.

The eyelids are among the most delicate structures in the face, composed of fine layers of soft tissue and the thinnest skin in the human body (eyelid skin being less than 1 mm thick, with sparse underlying subcutaneous fat). This relative absence of a padding fat layer predisposes to the characteristic skin laxity and fine wrinkles that appear with age.

 

Volume enhancing eyelid surgery (tissue-sparing blepharoplasty) 

Volume-enhancing upper eyelid surgery or ‘tissue-sparing upper blepharoplasty.’ Unlike traditional techniques which alter volume proportions between fat and muscle (and may have negative effects on ageing in the long term), this method reconstructs and maintains a person’s previous youthful appearance by repositioning rather than removing tissue. It is our In-House Plastic Surgeon, Dr.Mohd Ali’s opinion that this approach best considers the aging process in this area and delivers results that are the most rejuvenating and long lasting.

 

Lower Eyelid Surgery

Lower eyelids may suffer from loss of skin/muscle tone and fat accumulation over time, which can cause bulging beneath the eyes and hence the formation of eye bags. Surgery (known as lower blepharoplasty) helps rejuvenate lower eyelids by relocating or removing excess fat, and where indicated, tightening and strengthening loose or sagging tissue. The results are softened wrinkles, tightened skin and improved eyelid function.

The lower eyelid with associated Canthal Ligaments and Cartilage (Tarsal Plate) is among the finest and most complex of facial structures. It protects the eye and helps to keep the eyeball moist and healthy.

 

Muscle-conserving lower eyelid surgery

Canthopexy can be one of the most important and rejuvenative components of lower eyelid surgery. It is a surgical tightening of the outer corner of the eye, which may become loose or rounded with age. The procedure can restore the shape of the eye and improve the tension of the lower eyelid. Canthopexy is a tissue-conserving procedure that reinforces the existing tendon without cutting or detaching it. The restoration of tension within the tendon and muscles reduces the amount of fat that requires removal, which helps prevent the future development of lower eyelid hollowness.

Canthopexy is frequently combined with a light surgical tightening of the Orbicularis Oculi muscle, as improved muscle tone will help raise sunken eyelid tissue and soften the border between the eyelid and cheek.

An eyelid surgery (Blepharoplasty) is an operation to remove excess skin and fat from your eyelids and to tighten your skin and soft tissues around your eyes. During surgery, incisions are made in the natural folds of the eyelid, in the crease of the upper eyelid and just beneath the lashes or behind the lower eyelid. This way, incisions are virtually unnoticeable after they have healed.

While you are lying down during blepharoplasty, your surgeon will make precise markings to designate where excess skin and fat pads need to be removed. Some underlying muscle also may be removed. These tissues are removed with surgical instruments such as scalpels, surgical scissors, radio-frequency cutting devices and sometimes cutting lasers. During the procedure, your surgeon will make judgments about how much skin, muscle and/or fat to remove, based on a preoperative evaluation of factors such as your underlying facial muscle structure, bone structure and the symmetry of your eyebrows. Dry eye patients frequently require that less tissue be removed to avoid exposing more of the eye to the air, which can cause symptoms to worsen.

In cases where the eyebrows also are droopy, a procedure to elevate the eyebrows also may be appropriate. This procedure, called a brow lift.

WHAT DOES THE OPERATION INVOLVE?

The operation usually takes an hour to 90 minutes.

  • Upper-eyelid blepharoplasty – Your surgeon will make a cut on the natural skin crease just above your eyelid. They will remove any excess skin and fat that is pushing through the muscle.
  • Lower-eyelid blepharoplasty – Your surgeon will make a cut along the rim of your eyelid below your eyelashes to just beyond the outside edge of your eye. They will remove any excess skin, and remove or redistribute any fat that is pushing through the muscle.
