Water jet assisted liposuction is method of liposuction being used by cosmetic surgeons today. As in all types of liposuction, fatty deposits are removed from beneath the skin for a more contoured look. Water-assisted liposuction (WAL) may represent a new algorithm in the continuous evolution and development of liposuction techniques and devices to remove fat for contouring and harvesting purposes. WAL utilizes a dual-purpose cannula that emits pulsating, fan-shaped jets of tumescent solution, followed by simultaneous suctioning of the fatty tissue and the instilled fluid. A variable-force infusion pump drives the infiltration solution through a closed tubing system into a passageway within the application cannula. The fluid streams out from the cannula’s nozzle tip at a 30° angle to loosen the fatty tissue.

The washed-out fatty tissue is evacuated from surgical sites through a separate channel within the cannula, which is connected to an integrated suction unit. The application cannula vary in diameter, arrangement, and sharpness of openings. The flow rate of the infiltrate, as well as the application of variable intensities of negative pressure, can be selected at different levels, depending on the purpose.

A sterile container can be connected between the working cannula and the suction pump to collect the aspirate under reduced negative pressures. The fat is separated from the infranate for immediate usage without centrifugation. There is usually very little bruising or swelling and water jet assisted liposuction often has a very short recovery time, partially because the use of water means that there is less tissue trauma to the body.

WAL relies on two tumescent subcutaneous infiltration solutions. A higher concentration of lidocaine in the infiltration solution is employed during Phase 1 to provide longer-lasting local anaesthesia and vasoconstriction. A solution with lower lidocaine concentration is instilled during Phase 2 (simultaneous infiltration and aspiration) and Phase 3 (drying or finishing) to reduce the pharmacological effects of lidocaine and the accompanying fluid load.

Progressive improvements in the application of tumescent solutions, methodologies and instruments, and criteria for patient selection remain critical for patient safety and efficacy with any type of liposuction procedure. This preliminary report investigates whether saline rinsing of fatty tissue results in safe fluid and lidocaine management, minimal blood loss, consistent liposuction shaping with tissue accommodation, efficient fat harvesting, transfer and survival, and low complication rates with acceptable outcomes. Rhinoplasty modifies the functional properties and aesthetic appearance of the nose through operative manipulation of the skin, underlying cartilage, bone, and lining. The incision type that the surgeon uses classifies the rhinoplasty as open or closed.

In open rhinoplasty, the surgeon makes a small incision in the columella between the nostrils and then makes additional incisions inside the nose. Closed rhinoplasty involves incisions only in the interior of the nose.

 

What is a Rhinoplasty?

A Rhinoplasty is a cosmetic plastic procedure performed on the nose to change its structure, shape, and form, as well, improve simultaneously the airway flow and function, in order to improve nasofacial balance, function and overall aesthetic appearance.

 

Who is a candidate for a Rhinoplasty?

An excellent candidate for a Rhinoplasty procedure is a patient who is medically well and has realistic expectations and who understands the risks and benefits of the procedure and would still like to proceed. This can include individuals with excessively large noses, dominant bridge bumps, bridge scoops, long noses or short noses, crooked noses, overly-resected bridges or “bridge scoops”, as well as wide nostrils and bulbous tips.  All these nasal facial variants must have some desire to improve their naso-facial contour, balance or proportion.

 

A thorough physical examination

A thorough physical examination must be performed, both externally and internally. The inside exam, or endonasal exam is performed to diagnose any clinical or subclinical obstruction, deviation of the septum, with or without increased size of the turbinate bones.  If there is evidence of an airway obstruction, a Septoplasty, or partial or near-total removal of the obstruction septum, or turbinate’s, may be required at the time of the surgery. Dr.Mohd Ali will take the time to review your nose issues, so you can assess the range of nose improvement that can be achieved before you decide to proceed with the surgery.

 

How is a Rhinoplasty performed?

There are two broad categories of Rhinoplasty procedures performed by Dr. Mohd Ali and it’s done under Local Anaesthetic which means that the patient will be fully aware, alert and in conversation during the procedure.  Patients will benefit from Dr.Mohd Ali’s extensive 15 years of experience in performing Rhinoplasty surgery and all cosmetic and corrective surgeries from head to toe.

The first broad category is an external Rhinoplasty, where a small incision is made in the base of the nose and the nasal skin is lifted up over the cartilages and bony structure to optimally visualize and reveal the nasal anatomy so it can be structurally changed and stabilized for an excellent long-term result.

The second broad category is a closed Rhinoplasty, also known as Internal or Endonasal Rhinoplasty.  In a closed rhinoplasty, there is no small incision at the base of the nose, in what’s called the columella, but rather the incisions are placed entirely inside the nose and inside the nostrils.

An internal, or endonasal Rhinoplasty, as it is called, saves the patient the small incision in the skin of the partition of the nose (columella), and is excellent for dorsal bridge reductions and some modest tip and airway breathing concerns, but is usually a suboptimal approach for more complex procedures involving extensive or combined changes to the tip of the nose, balancing bridge and tip work or supporting the external valves of the nose and stabilizing all these structures so there is a predictable long-term result following your Rhinoplasty. For the majority of Rhinoplasty, the external approach is combined with internal incisions. This will give you the best possible cosmetic and functional outcome.

The three keys to a healthy weight:

 

  1. Eat well

  • Don’t skip meals – this may lead to overeating later. Weight loss efforts are most successful when meals and snacks are small and spread out during the day.
  • Eat balanced meals. Choosing a wide variety of foods is important for good health, but control portion sizes and avoid second servings. Try serving the foods on your plate with these tips in mind:
    • Fill one quarter of your plate with lean protein – this portion size fits within the palm of your hand. Lean protein sources include fish (especially salmon), poultry, egg whites and tofu.
    • Then fill at least half of your plate with non-starchy vegetables like tomatoes, salads and other greens, broccoli, carrots, mushrooms and green beans.
    • If desired, add a small portion of fruit or a cup of low-fat or non-fat milk or yogurt. Or, enjoy these as a between-meals snack instead.
  • Limit foods that are high in saturated fat, since fat is a concentrated source of calories.
    • Choose lean meats. Trim visible fat from meats and skin from poultry before eating.
    • Use lower-fat cooking methods, such as baking, broiling, roasting, stewing and steaming. Avoid frying foods.
    • Watch out for added fats from condiments, such as cheese, butter, margarine, mayonnaise and salad dressing.
    • Eat less added sugars in the form of table sugar, honey, sweets, pastries and desserts.
    • Avoid high-calories beverages like sodas, fruit juices and alcohol.

 

  1. Be active – exercise

Try making exercise a daily habit. Aim for at least 30 minutes of moderate activity, such as brisk walking, on most days of the week. If your goal is to lose weight, however, you’ll benefit from doing more, as much as 60 to 90 minutes of activity per day.

Don’t set the bar too high when you’re just getting started. Start slowly, and gradually increase your daily activity level. Some tips for increasing activity levels:

  • Limit TV, computer and video game use.
  • Schedule family exercise time.
  • Get an exercise video or equipment to use at home.
  • Walk the dog.
  • Do errands by foot or bike.
  • Take the stairs instead of the elevator.
  • Park farther away or get off the bus a few stops early and walk the rest of the way.
  • Join a walking group, aerobics class, sports team, local pool, gym or community centre.

To stay motivated

  • Choose a variety of activities to keep your workouts interesting.
  • Set realistic goals, at a reasonable pace.
  • Plan exercise into your weekly schedule.
  • Find an exercise buddy – you can rely on each other for support.
  • Keep an exercise log. Writing down what we do helps keep us focused.
  • Set a non-food reward for progress in meeting your goal.

 

  1. Be aware and plan

  • At home, eat only while sitting down at the table and stay out of the kitchen unless you are preparing food.
  • Keep tempting foods out of sight and make healthy snacks available.
  • Use smaller plates, bowls and glasses.
  • Make it inconvenient to eat more by chewing gum or brushing your teeth after meals.

 

Eat mindfully

  • Take small bites.
  • Chew your food well.
  • Put your utensil down between bites and savour the taste of the food.

 

Monitor your progress:

  • Check your body weight at least once a week – daily is okay – and keep a logbook.
  • Keep a daily food journal. It will make you think more carefully about what you’re eating and allow you to monitor calories. There are many ways to track calories, including applications for your handheld device, free or paid online programs, or the good old fashioned paper and pencil method.

How Liposuction Works

Liposuction is an outpatient procedure and can be safely performed using local anaesthesia, intravenous sedation, or local anaesthesia, depending on the nature of the procedure. Your cosmetic surgeon will recommend the anaesthesia method that is most appropriate for your needs.
Your cosmetic surgeon will make one or more small incisions near the treatment area. A cannula, or thin tube, is inserted through these incisions to loosen excess fat beneath the skin. Then, the fat is suctioned out. The entire procedure can take under an hour or up to 3 hours or longer, depending on the number of areas treated.

 

Liposuction Recovery

Today’s advanced liposuction techniques are designed to minimize swelling, trauma and discomfort, and when the procedure is performed by a qualified cosmetic surgeon, recovery from liposuction can be remarkably quick. Many patients return to work just a few days after their procedures, depending on the physical requirements of their jobs and the extent of their procedures. While your cosmetic surgeon can prescribe pain medication, most patients find over the counter options more than enough to manage any post-operative soreness.

Your cosmetic surgeon may give you a compression garment to wear for a few weeks following surgery. This helps to minimize swelling and encourage optimal skin contraction. With proper care, scars should fade significantly over the months following surgery; in many cases, scars are barely visible after a year or so.

 

Results can last permanently with proper care

Liposuction removes fat cells from your body, and these fat cells will not grow back. However, it is important to maintain a stable weight to ensure that your new look remains optimal. If you gain a significant amount of weight in the future, your overall appearance is likely to change as your body stores excess fat in other locations.
When performed by a qualified plastic surgeon, liposuction is a safe procedure and an excellent way to improve the shape of your body. The best way to decide if liposuction is right for you is to consult our qualified plastic surgeon.
Those keen in having a liposuction done must be aware on the safest method of liposuction and post liposuction issues, the safest and latest liposuction method is a technology called ‘Water Assisted Liposuction’ and must be done by a highly experienced Plastic Surgeon or General Surgeon and not a General Practitioner.

Once the procedure is done, patients will experience leakage for a week where excessive water mixed with fats and blood stains will be discharging and this is very much expected and common after a liposuction procedure. Patients will be called in for follow-ups for close check-ups and dressing. Swelling will take two weeks to subside and the results of fat loss will show in the first month. Full results will take at least four months to reflect.

The Compression garment is not an option but compulsory for 12 hours in a day for the next one month. Wearing your compression garment will help reduce the swelling and hasten the healing process. No exercises are allowed for one month after the procedure. Please keep in mind that final results may take six to twelve months and one is expected to live a very healthy lifestyle and maintain a strict diet after the liposuction procedure. Please take note that for those who experience loose skin after liposuction. Patients will be advised to purchase a course of Laser Skin Tightening sessions.

As with Fat Transfer procedures, this can be done immediately during the liposuction and yes, the full results will take four months to reflect and patients are advised to consume healthy fats such Avocados, Cheese and Full Cream Milk.

As for a Tummy Tuck procedure, it can only be done four months after the liposuction procedure. Patients must be aware that in most cases, a Tummy Tuck procedure would require a liposuction of the abdomen and flanks and then followed by a Tummy Tuck procedure four months later. Protocols mentioned must be strictly followed for safety to health and optimum results.

An Epicanthoplasty procedure aims to lengthen the inner parts of the eye to make the eyes bigger, brighter and more alert. It is Also called the “inner corner fold removal” or “Mongolian fold correction”, this is a cosmetic procedure done by Malaysia’s only Craniofacial Surgeon, Dr.Mohd Ali. He is a qualified Plastic Surgeon of 15 years and sub-specialised in Craniofacial Surgery, meaning an expert in the reconstruction of head, ears, eyes, nose, mouth, face and neck region.

An Epicanthoplasty is sought by women and men who would like to adopt eyes that not only “appear bigger, brighter, and more alert” but actually bigger physiologically. This procedure does not only focus on the inner eye fold, but also addresses the outer corner of the eyes. With lateral Epicanthoplasty, eyes can be significantly lengthened and slanted eyes can be corrected.

Epicanthoplasty is a cosmetic procedure that can be taken advantage by anyone who wishes to literally open their eyes wider than is physiologically possible. Specifically, Epicanthoplasty is an ideal procedure for patients, whether male or female, who:

Once a patient has fully decided on Epicanthoplasty, a pre-surgical evaluation is performed to assess the severity of the Epicanthal fold. Once assessed, the incisions are planned and marked. There are two different types of Epicanthoplasty, which are:

  1. Medial Epicanthoplasty – This focuses mainly on the inner corner of the eye, making the eye look bigger and more open; it can also straighten slanted eyes.
  2. Lateral Epicanthoplasty – This procedure does not only focus on the inner eye fold, but also addresses the outer corner of the eyes. With lateral Epicanthoplasty, eyes can be significantly lengthened and slanted eyes can be corrected.

 

Following a thorough assessment, the doctor will advise as to which procedure is best for the patient’s specific needs. The actual Epicanthoplasty usually starts with the application of local anaesthesia (LA).

Epicanthal surgery is usually performed on an outpatient basis and lasts about 30 minutes to an hour. After surgery, the eyelids would appear swollen and bruised. A recovery period of two weeks is often recommended. During recovery, the eyelids are allowed to heal and return to normal, so patients are advised to rest and refrain from strenuous activities and sports. Pain medication is prescribed to manage pain. Wearing eye makeup is discouraged until after the stitches have been removed which is usually after a week.

Can a person who lacks fats do the procedure from a fat donor?

The answer is a complete no. There is no such thing as a fat donor as the body will reject it hence it’s not an option. Such a person has to opt for implants. It’s pertinent that those who wish to undergo plastic surgery are fully aware that patience is core because patience is the foremost quality to have during the recovery period which can get frustrating and nerve wrecking. Recover period can be anywhere between two weeks to two months and in some cases up to six months. During this period of time many would feel that their post-surgery doesn’t seem to be healing or looks too ugly. This is the standard process of all those who undergo plastics surgery

Potential side-effects are most likely to occur immediately after your operation. Because of this, it is very important to follow all of your surgeon’s post-operative instructions. You will be supplied with a comprehensive packet of post-operative instructions. Be sure to follow these instructions to ensure a healthy recovery and eliminate potential risks. Never hesitate to ask your plastic surgeon specific questions about your recovery process.

 

Get Your Support System, ready! 

Not only will you need assistance after your surgery but you will also need a caretaker to take you to and from your procedure as you will not be able to drive. Also, you’re probably going to be out of commission for at least a week after a plastic surgery procedure. Inform your family and friends about your upcoming surgery, and make sure that you have a support system in place – people who can help you cook, clean, and do other tasks since you may be limited in the amount of activity allowed during your recovery period. The first 2-3 days after surgery will be the most difficult as you may experience uncomfortable soreness. It’s best to have someone in your home to take care of you during this time.

 

Be Ready To Take Time Off Of Work

You’re probably going to have to take a week or two leave for work after your procedure. This means you may have to plan your cosmetic surgery around your available vacation days.

 

Relax And Keep Yourself Occupied

When you’re recovering from plastic surgery, it’s important that you limit activity and relax. Let the healing process take place. If you try to rush things, recovery may take longer. Keep yourself occupied. Check a few books out from the library or download a few on your Kindle. Find a few new shows to watch on Netflix or Hulu – this time is a great chance for you to relax and take things easy for a little while.

 

Maintain a healthy diet, start your meal plan!

Due to limited activity, you may not be able to run to the grocery store and cook a big meal for yourself for a little while, so prep ahead of time. Remember, a nutritious, high-calorie diet helps in your recover and If you can, try to have friends and family prepare ready-to-eat meals for you, so that you can eat well without having to cook on your own.

 

Avoid Exercise or Over-exertion 

Until you’re cleared to do so by your doctor, avoid overexerting yourself, or intense exercise. Post-plastic surgery recovery involves a lot of healing from sutures and incisions – and excessive exercise can cause damage to wounds that are still healing.

 

Drink Plenty of Water

Adequate hydration is essential for the post-plastic surgery care. Drinking plenty of water helps replenish the fluids lost during surgery.

 

Avoid Excessive Sun Exposure 

Minimize your exposure to the sun. You want to avoid any sun damage to your skin or scar tissue by minimizing sun exposure, wearing sunblock, and wearing the appropriate clothing to protect against harmful UV rays.  Since you may be wearing compression garments, you also want to avoid excessive heat so you are not sweating in your garment. Too much sun can also dehydrate you, which certainly doesn’t!

After surgery, you will probably notice some swelling, bruising, and minimal scarring (depending on the surgery). Don’t let these discourage you. If you follow your plastic surgeon’s post-op instructions, you will start to see that these will dissipate – and you’ll see the full results of your plastic surgery procedure in no time. Just remember, recovery times will vary by surgery type. Everybody’s recovery process will be different.

Be patient when recovering from plastic surgery, and rely on your plastic surgeon and support network to get you through the process. You’ll be back on your feet in no time, feeling more confident and ready to reveal the new you!

Water Assisted Liposuction

Water assisted liposuction is the latest and safest method of liposuction being used by plastic surgeons today as it holds no risk as traditional liposuction. As in all types of liposuction, fatty deposits are removed from beneath the skin for a more contoured look. Liposuction is optimal for people who are in good physical shape.

Instead of injecting the body with tumescent fluid and removing the fat cells manually with a cannula, or using laser technology to melt and dislodge the fat, the water assisted liposuction actually uses a slightly pressurized stream of saline to dislodge fat and simultaneously remove the cells. Instead of destroying fat cells, this type of liposuction loosens the fat cells to facilitate more gentle removal. Water Assisted liposuction injects tumescent fluid (saline and medicines including a local anaesthetic) into the body part being treated. Tumescent fluid is proven effective and several alternate liposuction technologies are now using the tumescent technique for its local anaesthetic benefits. In the case of water assisted liposuction, it then uses different processes to actually remove the fat and manage fluid levels.

 

The Procedure

Once the local anaesthesia has taken hold, a cannula is then introduced into the tissue to irrigate and then simultaneously suction the loosened fat cell from the body. Finally, any remaining fluid is removed. The procedure is considered gentle and minimally invasive and the force seen in traditional liposuction methods is not needed when using jet-assisted liposuction. There is usually very little bruising or swelling and water assisted liposuction often has a very short recovery time, partially because the use of water means that there is less tissue trauma to the body.

 

No General Anaesthetic required

This is another liposuction technology that does not require general anaesthetic but only a Local Anaesthetic (LA), which drastically reduces risks to the patient and significantly reduces costs. Also, because of the fluid management used in this technique, the surgeon can do a more detailed anatomical assessment while conducting the procedure, so the results are excellent. Basically, the area of the body doesn’t swell up, so the plastic surgeon can actually see the results of the liposuction as he or she is doing it.

Upper eyelid surgery can rejuvenate puffy, sagging or tired-looking eyes by removing or redistributing excess fat, skin and muscle from the upper eyelid. Also known as upper blepharoplasty, it may be performed for purely cosmetic reasons, or to improve sight by lifting droopy eyelids out of the field of vision. The surgery aims to recreate a fresh, open and youthful appearance.

There is a wide range of possible procedures performed under local anaesthesia. Incisions are placed either in the natural lines above the eye, providing a quickly fading scar, or inside the lid, resulting in instant invisibility.

The eyelids are among the most delicate structures in the face, composed of fine layers of soft tissue and the thinnest skin in the human body (eyelid skin being less than 1 mm thick, with sparse underlying subcutaneous fat). This relative absence of a padding fat layer predisposes to the characteristic skin laxity and fine wrinkles that appear with age.

Volume enhancing eyelid surgery (tissue-sparing blepharoplasty)

Volume-enhancing upper eyelid surgery or ‘tissue-sparing upper blepharoplasty.’ Unlike traditional techniques which alter volume proportions between fat and muscle (and may have negative effects on ageing in the long term), this method reconstructs and maintains a person’s previous youthful appearance by repositioning rather than removing tissue. It is our In-House Plastic Surgeon, Dr.Mohd Ali’s opinion that this approach best considers the aging process in this area and delivers results that are the most rejuvenating and long lasting.

 

Lower Eyelid Surgery

Lower eyelids may suffer from loss of skin/muscle tone and fat accumulation over time, which can cause bulging beneath the eyes and hence the formation of eye bags. Surgery (known as lower blepharoplasty) helps rejuvenate lower eyelids by relocating or removing excess fat, and where indicated, tightening and strengthening loose or sagging tissue. The results are softened wrinkles, tightened skin and improved eyelid function.

The lower eyelid with associated Canthal Ligaments and Cartilage (Tarsal Plate) is among the finest and most complex of facial structures. It protects the eye and helps to keep the eyeball moist and healthy.

 

Muscle-conserving lower eyelid surgery

Though there are several approaches to the lower eyelid, the optimal strategy is to spare or redistribute tissue rather than remove it, and to retain fullness of the lid and the periorbital area.

For most cosmetic lower eyelid surgery, Dr.Mohd Ali employs the Transconjunctival approach, a technique which involves cutting along the inside surface of the eyelid rather than through the outer skin. Fat is then conservatively removed or repositioned to restore eyelid fullness. This method of access conserves the structural integrity of the eye muscle and nerves, leaving motor function intact. Furthermore, the healing process is enhanced and it results in no visible scarring of the skin.

Several options for treating loose skin may be considered along with Transconjunctival eyelid surgery. These include microsurgical skin trimming, skin peel or laser skin resurfacing.

 

Canthopexy

Canthopexy can be one of the most important and rejuvenative components of lower eyelid surgery. It is a surgical tightening of the outer corner of the eye, which may become loose or rounded with age. The procedure can restore the shape of the eye and improve the tension of the lower eyelid. Canthopexy is a tissue-conserving procedure that reinforces the existing tendon without cutting or detaching it. The restoration of tension within the tendon and muscles reduces the amount of fat that requires removal, which helps prevent the future development of lower eyelid hollowness.

Canthopexy is frequently combined with a light surgical tightening of the Orbicularis Oculi muscle, as improved muscle tone will help raise sunken eyelid tissue and soften the border between the eyelid and cheek.

Rhinoplasty procedures are one of the most common plastic surgery procedures as it significantly contribute to image. However, for those who desire adjustments to the nostrils alone, an Alarplasty procedure can be performed under local anaesthesia and result in a dramatic aesthetic transformation for the patient.

Here at Glojas, an Alarplasty and Rhinoplasty is done on Local anaesthesia which means it’s much safer and lower in costs. Patients can undergo both procedures on LA and can be performed as an individual surgery simultaneously. An Alarplasty is commonly combined with open, closed, and ethnic rhinoplasties.

Once the patient is thoroughly anesthetized the procedure will begin. Preoperative marks will determine the amount of nasal tissue to be removed. Depending on the patient’s anatomy, the shape of the tissue to be excised will typically be in a triangle, rectangle, or diamond shape. The surgeon will make an incision just above the outside crease where the nostril connects to the cheek. The incision will continue to the inside of the nose, following all the way around the preoperative markings. The tissue will then be excised. Sutures will then be used to reattach the nostril to the base of the nose in a more narrow position. The procedure will be repeated on the opposite nostril. Incisions and resulting scars are well hidden within the natural creases of the nose.

The goal of an Alarplasty procedure is to reduce the size of the nostrils for a more symmetric nose or more balanced facial features.
